Case Statement
Press kit about Grace Manor Health Care Facility Grace Manor Health Care Facility is a culturally diverse, skilled nursing facility with a rich heritage of excellence in personalized care. Our mission is to provide for the needful elderly and those underserved in our community with quality care in a safe and comfortable environment. Conveniently located in the heart of Buffalo and on local bus routes, families, employees and other visitors find Grace Manor easily accessible.

While visiting infirmed and housebound members of his congregation, Rev. Houston Williams, Pastor of Grace Tabernacle Church of God in Christ located at 579 Williams Street in Buffalo, witnessed many examples of unhealthy conditions for the needful elderly in the community. He presented these findings to his congregation and received unanimous support for the establishment of a facility to care for the elderly and infirmed in need.

In 1979, the challenging road toward the establishment of Grace Manor Health Care Facility was undertaken as Pastor Williams began fundraising activities and solicited donations from church members and the community at large. Despite Pastor Williams’ ill health, major fundraising activities and the endorsement of area faith-based communities helped meet projected goals. His death in 1990 shadowed the success of the project but on January 16, 1998, the day that Grace Manor welcomed its first resident, Pastor Williams’ dream became a reality. Grace Manor became the first African-American nursing home in New York State outside of New York City.

With the mission of offering culturally competent continuing care to our city’s communities of need, Grace Manor has developed a sensitive care model in which traditions are honored in order to enhance the caring environment. Grace Manor strives to respond to the richly varied values, beliefs and perspectives of its residents. Its urban location also allows the families of these residents easy and frequent access to their family members.

In its 167-bed facility, Grace Manor specializes in long-term care and short-term rehabilitation. A personalized care plan is developed for each long-term resident based on needs as determined by a health team evaluation. Communication with the resident’s support network is underscored as this care plan is reviewed regularly with the resident, caregivers and family members and modified as needed. Grace Manor has also developed a highly skilled array of rehabilitation specialists to manage the short-term health care needs of those requiring this service. The goal is to offer professional rehabilitation designed to promote safe and effective healing.

Grace Manor’s location at 10 Symphony Circle allows its residents and employees to live and work in a warm and compassionate city location convenient to homes, families, churches, transportation and community life. Grace Manor strives to belong to this vibrant urban community that hasn’t forgotten its elderly in need.
