Admissions at Grace Manor Health Care Facility
When you or your family member decides to come to Grace, our professional staff helps you though this process, whether for a short-term stay or a long-term admission.

Admissions at Grace Manor Health Care Facility Step 1: Apply
Grace admissions staff provides you with an application form that helps gather your personal information. We also request copies of:
  • Medical Insurance Cards
  • Health Care Proxy or Living Will, if you have one
  • Financial Statements, if private pay
Step 2: Provide Medical & Case Information
Next, we need to understand your medical condition. If you are joining us from home, Grace Manor needs a Medical Form that is completed by your physician. This is automatically done for you if you are coming from a hospital or other facility.

Admissions at Grace Manor Health Care Facility Also, if you are coming from home, we need a Patient Review Instrument and State Assessment Form. In the hospital, these forms are handled by the hospital’s Discharge Planner or Social Worker. At home, you may need to call one of the following agencies to perform the assessment, and there may be a change if Medicaid does not cover the cost:

  • McAuley Seton Home Care: 716.685.4870
  • If you are active with Community Alternative Systems Agency (CASA), contact your caseworker.
Once your paperwork is received, we work with you to determine if Grace Manor is an appropriate fit for providing your care.

Step 3: Get Ready to Enjoy Life at Grace
Admissions at Grace Manor Health Care Facility Welcome! Once you are accepted, you will want to plan for a seamless transition into Grace. Our admissions staff can discuss the following:
  • Identify appropriate clothing
  • Select personal electronics
  • Rental equipment (e.g., walkers) returned to the rental company
  • Notify family members
  • Sign or Decline advanced directives, such as:
    • Living Will
    • Do Not Resuscitate
    • Feeding Restrictions
    • Medication Restrictions
    • Other Treatment Restrictions
    • Health Care Proxy
    • Organ Donor
