When you or your family member decides to come to Grace, our professional staff helps you though this process, whether for a short-term stay or a long-term admission.
Step 1: ApplyGrace admissions staff provides you with an application form that helps gather your personal information. We also request copies of:
Next, we need to understand your medical condition. If you are joining us from home, Grace Manor needs a Medical Form that is completed by your physician. This is automatically done for you if you are coming from a hospital or other facility.
Also, if you are coming from home, we need a Patient Review Instrument and State Assessment Form. In the hospital, these forms are handled by the hospital’s Discharge Planner or Social Worker. At home, you may need to call one of the following agencies to perform the assessment, and there may be a change if Medicaid does not cover the cost:
